There are times when a patient or family may want to change
hospice providers. In this episode we explain the process to
follow for that to occur.
Here are some things to remember:
patient or family has the right to transfer from one hospice agency
to another agency of their choice.
transfer can occur one time in a benefit period (hospice agency is
responsible for tracking this, and letting the patient/family
are several reasons for transfers - dissatisfied with care,
personal connection with someone at another agency, moving to an
agency that has a freestanding inpatient unit.
