In the latest survey (2016), almost 42% of hospice patients were receiving hospice care in a long-term care facility. In today’s episode you can learn how the hospice agency and the long term care facility coordinate care for a hospice patient.

98% of hospice patients receive hospice care while living at home, but “home” can be defined in many ways: a private residence, an assisted living facility, a group home, a nursing home, a private care home, or foster home.  Many people receiving hospice care live in a long term care facility such as a nursing home, or assisted living facility. The hospice agency staff and the long term care facility staff work together to provide care for the hospice patient.
